Nomination Instructions

The Diller Teen Tikkun Olam Awards recognize Jewish teen volunteers, 13-19 years old. Teens remunerated for their services are not eligible.

Teachers, rabbis, community leaders, or individuals who know the value of the teen’s volunteer service, may submit nominations. Teens may self-nominate. Family members are excluded.

It is not possible to save your work mid-way through the form. To ensure your nomination is not lost due to unforeseen circumstances, we suggest that you prepare and save a copy of your form in a separate document and then paste your answers into the fields below.

All fields are required. Put “N/A” or "00" for any question not applicable.
